近日,老编收到一篇某大社披露的PSC热门滞留案例,本期整理后推送给大家,供各位参考,有则改之,无则看看热闹。
案例一:主甲板空气管或者通风筒损坏、洞穿等
缺陷描述:Plenty of ventilators and air pipes on main deck holed or damaged.
缺陷依据:载重线公约LL66/ANI/R19 ,R20
缺陷举例: All ventilators for NO.1, NO.2 and NO.3 cargo holds stuck in open position(1号、2号、3号货舱通风筒被卡在打开位置).
案例二:舱盖不能保持风雨密
缺陷举例:No.1 hatch cover can not be closed properly due to the front edge of the No.1 hatch cover being deformed,and total 8 pcs of hatch cover cleats missing(由于1号舱盖前端变形,8个锁紧装置缺失,导致1号舱盖不能关严).
缺陷依据:载重线公约LL66/ANI/R14-货舱口及其他舱口
缺陷举例2:Ventilators and auto cleats for all hatch covers found damaged and rusted seriously(货舱通风机损坏严重,锁紧装置锈蚀严重).
A large number of clamping devices for NO.3 cargo hold not installed while vessel underway from ** to ** port(3号货舱的舱盖锁紧装置没有安装)
缺陷依据:LL66/ANI/R16-风雨密钢质舱盖或其他相当材料舱盖所封闭的舱口
案例三:甲板安全通道被堵
缺陷描述:Vehicles were loaded and secured on both sides of passage way on main deck which blocked the safe passage for crew before departure(甲板上的安全通道位置被装载了汽车,严重影响船员安全通行).
缺陷依据位于载重线公约附则I的第25.4条,这一条要求:Satisfactory means for safe passage required by regulation 25-1 (in the form of guard rails, lifelines, gangways or underdeck passages, etc.) shall be provided for the protection of the crew in getting to and from their quarters, the machinery space and any other spaces used in the essential operation of the ship.
也就是船舶甲板上船员安全通道布置应符合第25-1条“为船员安全通道采取的措施”的要求,25-1比较细,大部分散货船采用不小于10mm的钢丝安全绳+不大于10m的撑柱,或舱口围板上有支撑的单根扶手或钢丝绳,上图的这个,把通道完全堵掉了,显然是不行的。
案例四:应急消防泵故障及隔离阀
缺陷描述:The discharge pressure of emergency fire pump failed to build up during the PSC inspection. Furthermore, ship's crew deliberated to switch on the mainfire pump several times when the PSCO call for the testing on the emergency fire pump(PSC检查期间,应急消防本启动后压力不足,两股水柱不能达到公约要求水压(6000吨以下0.25N/mm2以上,6000吨以上0.27N/mm2以上),船员将消防泵启动,试图造假).
缺陷依据:SOLSA公约/CII-2/R10.2.2.3
缺陷描述:The interval of two isolating valves of the fire main fitted on the main deck measured at about 53meters in length,which exceeded 40 meters as required by the SOLAS(主甲板上的两个消防隔离阀之间的距离是53米,超过了SOLAS要求的40米的要求)
隔离阀的事,老编讲过多次,缺陷依据位于SOLASII-2第10.2.1.4.4条- 在液货船上,隔离阀应装设在尾楼前端消防总管受保护的位置处,其在液货舱甲板上的间距应不超过40m,以在发生火灾或爆炸时维持消防总管系统的完整性。
这种缺陷属于船检责任,PSC会追究RO责任,整改起来也很麻烦,首先要修改消防控制图,然后再增减一个隔离阀,然后重新试压检验。
缺陷描述:Discharging water pressure of EM’CY fire pump on main deck is not enough while the fire main isolating valve is closed, furthermore, by opening one fire hydrant in E/R, it is found that water pumping out constantly from that hydrant, it is indicating that the isolating valve on fire main malfuntion(消防管线隔离阀关不严,从照片上看,PSC是从机舱放水,这么测试不科学).
案例五:机舱水喷淋故障
缺陷描述:The fixed water mist system in front of the combined boiler was tested during inspection, unfortunately no water mist is ejected at the time(PSC检查期间,让船员测试锅炉喷油器上方的局部水基系统是否好用,结果没喷出来水).
缺陷依据位于SOLAS公约II-2第10条,500总吨及以上的客船和2,000总吨及以上的货船,并且容积超过500立方米的A类机器出所,要配这玩意,PSC的常规检查手段是,让船员测一测,喷一喷,如果不喷水,这东西就成了完蛋的玩意,要被滞留了。
案例六:货舱抽烟探测系统故障
缺陷描述:Smoke detection system defective: Airflow line No2-4 found
fault during the inspection(PSC检查期间,发现该轮货舱抽烟探测系统的2号、3号、4号舱故障).
缺陷依据位于SOLAS公约II-2第19条对与危险货物运输的特殊要求:滚装处所应装设1个符合《消防安全系统规则》的固定式探火和失火报警系统。所有其他类型的货物处所应装有1个符合《消防安全系统规则》的固定式探火和失火报警系统或1个符合《消防安全系规则》的抽烟探火系统。如果安装了1个抽烟探火系统,应特别注意到《消防安全系统规则》第102.1.3条的要求,以防止有毒烟气泄漏到有人区域。
持有危险货物适装证书的船舶才要装这玩意,常见类型的工作原理是:位于CO2间的抽风机马达启动,开始从货舱抽风,通过CO2管路至三向阀,然后到位于接线箱内的聚烟器,聚烟器对抽得的气体进行探测,超过浓度要求发出警报,同时在主控制面板及复示器面板显示,如下图:
案例七:装货相关
缺陷描述:Numerous lashing pad eyes found rusted and banding seriously on each cargo hold,the thickness of partly pad eyes ever less than 4 mm,and numerous of corner casting found rusted and damaged seriously(集装箱的系固装置损坏严重).
缺陷依据位于SOLAS第VI章第5条-积载和紧固
缺陷描述:Lack of securing plan according to Cargo securing manual while vessel loading general cargo at this port(船舶装货前,没搞货物绑扎计划).
案例八:演习使用同一个照片
缺陷描述:The same photos used as the evidences for conducting abandon ship drills on 27.FEB,21.MAR, 12.JUN and 28.JUL,which appears drills did not conduct as real as possible(多次演习使用同一张照片).
缺陷依据:SOLAS公约III章第19条
案例九:火灾探测报警故障
缺陷描述:The Zones of No.9(ENG‘RM 3RD.DK) and No.11(ENG’RM CASING) of fire detection and alarm system were shut off during inspection,and the mentioned zone can not work normally,however the
Electrician in charge of repairing the fire system was not be noticed untill PSCOs pointing out(火灾探测报警9号区和11号区故障,被电机员屏蔽,PSCO检查时,电机员没有主动告知).
缺陷依据:SOLAS公约II-2章第13条.
案例十:二氧化碳管线锈蚀穿洞
缺陷描述:The release pipe of fixed CO2 fire extinguishing system for No.3 cargo hold (aft,port side) corroded and broken. The control air pipe of the main release valve for engine room was disconnected during inspection(3号货舱二氧化碳释放管线锈蚀断裂,机舱二氧化碳释放控制管路没有与主释放阀连接).
案例十一:柴油机燃油报警故障
缺陷描述:Oil leakage alarm for high pressure fuel lines of No.1 & No.3 A/E malfuncaton since 3 Oct. ,and the current situation has not reported to neither the local port authority nor the flag state/RO before arrival(自10月3日以来,1号和3号辅机燃油管路泄漏报警故障,并且船上没有向PSC、船旗国和RO报告).
缺陷描述:The jacketed high pressure lines and oil leakage alarm for main engine failed to test(主机高压油管溢油报警测试失败)
缺陷依据:SOLAS公约II-2章第4.2.2.5.2条,这一条修正较多,PSC用的时候要谨慎一点。
案例十二:驾驶台舵机报警故障
缺陷描述:Alarm system for steering gear at engine control room and bridge malfunction(位于机舱和驾驶台的舵机报警故障)
缺陷依据位于:SOLAS公约II-1章第29条
案例十三:锅炉报警故障
缺陷描述:Low level and low low level alarm of boiler water defective(锅炉低水位报警故障).
缺陷依据位于:SOLAS公约II-1章第32条
案例十四:应急操舵演习造假
缺陷描述:Emergency operation of steering gear drill was conducted on 11st -Dec, and recorded in the log book that the steering gear alarms was tested at that time. However, there is no such alarm can be emonstrated by the alarm history, the subject drill record has not reflected the real situation(这个缺陷PSC查的比较深,根据舵机演习记录12月11日船舶开展了一次演习,但是PSC调取了机舱报警记录,报警记录显示那一天舵机没有报警,由此确定船员演习造假。)
缺陷依据位于:SOLAS公约V章第26条,这一条要求每三个月要搞一次应急舵演习,PSC的这个查法,值得关注!
