1. Anterior fibres: These contribute to hip flexion and internal rotation. Tension in this area can refer pain along the iliac crest or to the front of the hip, sometimes mimicking pain from the hip joint itself.
2. Middle fibres: This section is primarily responsible for hip abduction. Tension here can cause pain along the lateral aspect of the hip, extending down the outside of the thigh towards the knee. It is often mistaken for IT band syndrome or trochanteric bursitis.
3. Posterior fibres: These contribute to hip extension and external rotation. Tension in the posterior part of the gluteus medius can create referral pain toward the sacroiliac region or into the lower back.
