Background and aims

Methods

Conclusion

Objective
关于小而密低密度脂蛋白胆固醇(sdLDL-C)和绝经状态的研究仍然很少。本研究的目的是评估中国女性人群血清sdLDL-C水平与不同绝经状态之间的关系。
Methods
研究人员于2022年进行了一项横断面研究,包括电子标准化问卷调查,人体测量和基于社区的生物标本检查。招募了居住在浙江省两个社区并于5月26日至9月17日参加社区健康检查的30-69岁常住成年人。根据绝经状况,符合条件的女性受试者分为绝经前,围绝经期和绝经后。采用SAS软件建立Logistic回归模型,探讨sdLDL-C水平与不同绝经状态的关系。
Results
共纳入2062例女性受试者,中位年龄为57(51,63)岁。绝经前451例(21.9%),围绝经期87例(4.2%),绝经后1524例(73.9%)。中位sdLDL-C水平为0.937(0.685,1.209)mmol/L,绝经前,围绝经期和绝经后女性的sdLDL-C水平呈逐渐显著上升的趋势,并且在绝经后女性中达到峰值。Logistic回归分析显示,在控制混杂因素后,与绝经前相比,绝经后与sdLDL-C浓度升高显著相关(OR=1.514,95%CI 1.025-2.238),而血清sdLDL-C与围绝经期(与绝经前相比)或血清sdLDL-C与绝经后(与围绝经期相比)之间均无显著相关性。
Conclusions
sdLDL-C水平升高与绝经后显著相关,与年龄无关。该研究支持sdLDL-C是绝经过渡的有希望的风险生物标志物。未来的研究应该考虑更广泛的人群和更严格和彻底的研究设计,以验证这些发现。

Background
多环芳烃(PAHs)是一类持续存在于环境中的大型有机化合物,被归类为持久性有机污染物(POPs)的重要子集。本研究旨在评估中国海口市孕妇和新生儿的PAH暴露情况,并评估其对糖脂代谢的影响。
Methods
在2022年5月至2023年12月期间,共选择了300例孕妇及其新生儿进行研究。收集生活方式和饮食习惯的数据,并使用气相色谱-质谱法测量静脉血和脐带血中的PAH水平。此外,还测量了糖脂代谢指标,包括空腹血浆葡萄糖(FPG)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白(HDL)和低密度脂蛋白(LDL)。进行乐相关和回归分析以探索PAH暴露与代谢指标之间的关系。
Results
在母体和脐带血中检测到13种多环芳烃。母体血液中∑PAHs的中位浓度(检测到的16种PAHs的总浓度)为11.211 μg/L,新生儿为10.921 μg/L。PAH暴露与糖脂代谢之间存在显著相关性,PAH暴露与TC和TG水平降低有关。靠近道路,烹饪燃料类型和饮食习惯等因素影响PAH水平。较高的教育水平与较低的PAH浓度有关,而住在道路附近和使用天然气作为烹饪燃料会增加暴露。
Conclusion
受环境和生活方式因素的影响,海口孕妇和新生儿存在明显的PAH暴露风险。怀孕期间接触PAH可能会影响母亲和新生儿的糖脂代谢,这突出表明需要采取干预措施来减少暴露。

目前尚不清楚运动诱导的刺激如何影响并转化为怀孕期间的免疫代谢适应。之前发现运动会影响母胎循环细胞因子(IL-1β、IL-6、IL-8、IL-10、TNF-α)。本项研究调查了i)怀孕期间运动训练计划对母体,脐带动脉和静脉血清中代谢标志物(血糖和脂质标志物以及C反应蛋白)的影响;ii)这些细胞因子是否介导了运动对代谢标志物的影响。将88例孕妇分为运动组(i n/i 44)和对照组(i n/i 44),参加了这项准实验研究。运动组每周3天进行60分钟的同时(有氧+阻力)运动训练。使用标准生化方法和Luminex xMAP技术在第17周和第34周以及出生时测量血糖和脂质标志物以及C反应蛋白浓度和细胞因子水平。总体而言,运动在怀孕期间并未引起母体代谢标志物的可检测变化,除了那些IL-8水平升高的运动者,其与母体总胆固醇降低有关(间接效应=-9.1;95%CI -24.6至-1.1)和低密度脂蛋白胆固醇增加(-8.9;-21.9至-1.1)。这表明运动可以优化脂质代谢调节的机制。此外,运动与较低的脐带动脉血糖水平有关。进一步的研究,特别是关于胎儿代谢的研究是必要的。

Expert opinion
