In this issue of Neurology®, Ghozy et al. report the results of a systematic review and meta-analysis investigating the sensitivity and specificity of cardiac CT compared with echocardiography (transthoracic echocardiography and transesophageal echocardiography combined) for intracardiac thrombus detection in patients with and without ischemic stroke.

In this issue of Neurology®, Guan et al. published a study in which they performed a secondary analysis of the INSPIRES trial with a specific focus on looking at the time-specific benefit and risk of dual antiplatelet therapy vs aspirin alone in patients presenting with a high-risk TIA or minor ischemic stroke within 72 hours who also had evidence of atherosclerosis.

Among patients with mild ischemic stroke or high-risk transient ischemic attack of presumed atherosclerotic cause, the net benefit of clopidogrel-aspirin initiated within 72 hours of symptom onset was pronounced in the first week and continued to a lesser degree in the following 2 weeks, outweighing the low, but ongoing hemorrhagic risk.

脑小血管疾病不断扩大的临床影响:微小但强大
The Expanding Clinical Impact of Cerebral Small Vessel Diseases:Tiny but Mighty (PMID: 39321405)
该研究证实了先前已知的脑白质高信号与一些不良临床结果的关联,包括抑郁、淡漠、步态障碍和睡眠障碍,这一研究结果支持了目前关于脑小血管病与广泛功能障碍有关而不仅仅是卒中和认知功能障碍的观点。
The study confirmed previously known associations of white matter hyperintense singals with several adverse clinical outcomes, including depression, apathy, gait disturbance, and sleep disturbance, supporting current understanding that cerebral small vessel diseases is associated with a broad range of impairments beyond stroke and cognitive dysfunction.
