Background
抑郁症与心脏代谢疾病并发的现象十分普遍,而且会增加死亡风险。然而,抑郁症患者的心血管代谢疾病的患病率和多病症趋势尚不明确。
Methods
分析了2005-2020年全国健康与营养检查调查中≥20岁抑郁症患者的数据。Joinpoint回归分析用于估计血脂异常,高血压,糖尿病,慢性肾脏疾病,非酒精性脂肪肝病和心血管疾病以及其中≥3种疾病的患病率趋势。在应用倾向评分加权后,使用泊松回归评估这些疾病在抑郁症与无抑郁症中的患病率差异。
Results
共纳入3412例患有抑郁症的成年人。在2005年至2020年的总体样本中,心脏代谢疾病以及≥3种疾病的患病率保持较高且稳定(趋势P>0.05)。2017-2020年,心血管疾病患病率为17.1%(95%CI 12.7%-21.5%),血脂异常患病率为58.4%(95%CI 50.4%-66.3%);40.7%(95%CI 34.4%-46.9%)有≥3种疾病。患有抑郁症的成年人的糖尿病,心血管疾病和≥3种疾病的患病率比没有抑郁症的成年人高23%-85%。
Limitations
使用自我报告的数据和/或一次性实验室测量可能会对参与者进行错误分类。
Conclusions
心脏代谢疾病的患病率很高,在美国成年抑郁症患者中,多发病率很常见。解决抑郁症心脏代谢疾病的预防,治疗和管理需要更多的公共卫生和临床关注。

Background
家族性高脂血症(家族性高胆固醇血症,FH)是一种常染色体遗传疾病。它包括类型杂合性家族性高脂血症(杂合性家族性高胆固醇血症)。HeFH主要由LDLR、APOB和PCSK9基因突变引起,以血浆低密度脂蛋白胆固醇水平升高为特征。
Methods
研究人员提出了一例归因于APOB基因突变的HeFH病例。从先证者及其父母的血液中提取外周血的全基因组DNA,并通过高通量测序对外周血的外显子进行测序。通过使用Sanger方法进行测序来验证所选突变位点。
Results
在先证者和母亲中均鉴定出APOB基因第26外显子(Chr2-21233189)的杂合突变c.6551A>G(p.Y2184C)。结合临床特征,最初考虑了由该突变引起的HeFH。
Conclusions
对于临床上高度怀疑FH的患者,应通过基因检测确定明确的诊断,使患者能够接受早期治疗并有效预防心血管事件的发生。
