哮喘患者可以发生持续气流受限，不仅是严重哮喘，相当比例的较轻的哮喘患者也可以有。对这样的患者应谨慎诊断慢阻肺，在我国2021版慢阻肺指南的诊断流程中，戴帽前还有一步，“排除其他诊断”。如果没有吸烟等危险因素暴露、没有肺气肿，肺功能符合哮喘病理生理和临床过程，有什么理由诊断慢阻肺呢？

这项研究的亮点在于分析了获益亚组和预测因素，有助于含ICS吸入治疗的个体化应用。非常有意思的是再一次验证了规范坚持治疗的重要性。另外，就EOS 100-300的灰区怎么结合临床，给出了量化指标。再者，ICS怎么选，对高剂量say no，倾向抗炎作用不是最强的品种：（1）验证了ETHOS研究降中重度AE，低剂量三联≈高剂量三联>高剂量ICS/LABA；（2）支扩剂的backbone地位并没有变。BMI太低的患者，没有保护作用，则可能和肺炎发生风险升高有关。对于ICS在慢阻肺中的应用，老文章还没有过时：长期使用ICS的潜在肺炎风险，你怎么看？

大多数慢阻肺患者在肺部康复（PR）后未能维持运动训练（PR的强度的确不太能坚持），那么在家进行最低限度的设备力量训练是不是也有获益呢？这项研究尽管主要终点呼吸困难没有改善，但是做做运动至少心情能好啊，体能表现也好一些。
