一直说既往急性加重是最重要的急性加重预测因子，那么为什么会发生这些急性加重？首先，要鉴别是否真的是急性加重，由于是回顾性判断，很难区别仅仅是一次呼吸道感染，还是感染诱发了急性加重，还是其他诱因所致的急性加重，或者根本是肺外问题导致的呼吸系统症状加重（比如心血管疾病）等等；其次，急性加重的原因，是下呼吸道致病微生物负荷升高，是气道炎症水平升高，是胃食管返流等合并症没有控制好，还是药物没有好好用？等等。不同的原因应该有不同的应对方式，ICS一招鲜显然有点过于简单了，有时甚至南辕北辙。
