4.防污染
食用油非法排放入海。 Cooking oil is discharged into the sea illegally.
油水分离器舱体损坏:漏水。 The trunk of the oily water separator is damaged: There is water leakage.
油水分离器故障 / 15PPM 报警监测器有缺陷。 The oil water separator is found out of order / The 15PPM alarm monitor is defective.
由于唯一的鼓风机故障,污水处理装置失灵。 The sewage treatment is out of order due to the only air blower malfunction.
15ppm 舱底水装置记录缺失。 The record of the 15ppm bilge device is not available.
船舶在上海排放污泥,但油类残余物处理证书是假的。 The ship discharged sludge at Shanghai, but the certificate of disposal of oil residues is fake.
水手长储物舱舱底水高位警报未测试(负责人不知道 30 秒延迟)。 The bosun store bilge well high level alarm fails to test (the person in charge does not know the 30 - second delay).
15ppm 舱底水警报未激活且在采样水旋塞阀关闭时舱底水排放未自动停止;油分计无法监测 15ppm 舱底水分离器的排放物。 The 15ppm bilge alarm is not activated & the bilge discharge is not automatically stopped with the sampling water cock valve closed; the oil content meter is unable to monitor the effluent from the 15ppm bilge separator.
油水分离器电磁阀故障。 The solenoid valve on the oily water separator malfunctions.
船员不熟悉油水分离器操作方法且无法进行测试。 The crew is not familiar with the operation method of the oily water separator and cannot proceed with the test.

6. 国际载重线公约(ILL)
许多舱口盖橡胶密封垫损坏和缺失。 A lot of rubber packing of the hatch covers is damaged and missing.
货舱舱口盖和舱口围板腐蚀。 The cargo hold catch cover and hatch coaming are corroded.
货舱舱口盖在航行中未有效固定。 The cargo hold hatch covers are not effectively secured during the voyage.
货舱鹅颈通风管顶部腐蚀穿孔。 The goose neck vent pipe for the cargo hold is corroded and holed on the top side.
货舱因碰撞损坏进水。 The cargo hold is damaged and flooded due to collision.
水手长储物舱小舱口损坏腐蚀。 The small hatch of the bosun store is damaged and corroded.
水手长储物舱远程排水系统故障(驱动线堵塞)。 The dewatering remote control system for the bosun store is out of order (the driving line is clogged).
舱口盖和舱口围板生锈穿孔。 The hatch cover and hatch coaming are rusted and holed.
通风机卡在开启位置。 The ventilator is stuck in the open position.
驾驶室进水系统故障。 The Water Ingress System in the wheel house malfunctions.
舱口盖锁定销卡住。 The hatch cover locking pins are seized.
舱口围板高度(450mm)和货泵舱门槛高度(550mm)不符合 LL66/P88/ANNX 1 要求的 600mm。 The heights of the hatch coaming (450mm) and the door sill to the cargo pump room (550mm) are not 600mm as required by LL66/P88/ANNX 1.
进水报警传感器故障。 The water ingress alarm sensor is defective.
顶边舱空气管浮子卡住。 The air pipe floats for the topside tanks (TST) are seized.

7.证书和文件
前任船长 / 大副 / 二副 / 轮机长 / 大管轮持有无效适任证书。 The former captain /chief officer /second officer /chief engineer /second engineer holds an invalid certificate of competency.
由于前任船长 / 大副 / 二副 / 轮机长持有无效适任证书,配员不符合船舶最低安全配员证书要求。 The manning does not meet the requirement of the ship’s minimum safe manning certificate due to the former captain /chief officer /second officer /engineer holding an invalid certificate of competency.
船上轮机长适任证书被发现是假的。 The certificate of competency of the Chief Engineer on board is found to be fake.
油污损害民事责任保险或其他财务担保证书过期。 The certificate of insurance or other financial security in respect of civil liability for oil pollution damage has expired.
在港口国监督检查期间船上没有国际能效证书(IEEC)/ 国际防止空气污染证书(IAPP 证书)。 The IEEC / IAPP Certificate is not available on board during the PSC inspection.

简易型航行数据记录仪故障报警(舱盒连接错误)。 The S-VDR fails the alarm (Capsule connection error).
航行数据记录仪数据存储单元连接故障报警。 The connection of the DRU (data storage unit) of the VDR (Voyage Data Recorder) fails the alarm.

船上没有最新的当地海图。 The latest local chart is not available on board.
电子海图(ENC)过期 / 未更新。 The Electronic Navigational Charts (ENC) have expired / Not updated.
失控灯布置不符合国际海上避碰规则要求(不可见角度扇区超过 6 度)。 The arrangement of the NUC lights does not comply with the requirements of COLREG (the invisible angular sectors are more than 6 degrees).
应急无线电示位标(EPIRB)电池过期。 The EPIRB Battery has expired.
船舶穿越军事禁区。 The ship crosses the military prohibited area.
日间信号灯开关故障。 The Day Signal Light Switch malfunctions.
船舶自动识别系统故障且未向港口当局报告。 The AIS malfunctions and does not report to the port authority.
雷达PM测试失败。 The RADAR PM Test fails.
9.国际船舶和港口设施保安规则(ISPS)
船舶通道失控(舷梯无人值守)。 Access to the ship is found out of control (No one is on duty at the gangway).
10.其他
应急职责数量与实际不符。 The number of emergency duties is different from the actual ones.
应急发电机有缺陷(电机烧毁、应急停止功能故障、电池问题等)。 The emergency generator is defective (motor burn-out, emergency stop function defective, battery problem, etc.).
检查期间三台发电机无法启动。 Three generators fail to start during the inspection.
应急发电机在 30 分钟内启动失败超过 3 次。 The emergency generator fails to start more than 3 times within 30 minutes.
两个集装箱未按照《国际海运危险货物规则》进行妥善隔离。 Two containers are not properly segregated according to the IMDG Code.
部分空气管、消防管和测深管腐蚀。 Some air pipes, fire pipes and sounding pipes are corroded.
