选择合适的骨隧道点和走形； 若自体肌腱过小或取腱部位损伤，需要选择同种异体肌腱或人工韧带进行重建； 若采用自体腘绳肌腱进行关节镜下 ACL 重建术，肌腱直径宜大于 8 mm。
股骨侧的固定方式一般有悬吊钛板、界面钉、横穿钉等。 胫骨侧的固定方式一般有外口界面钉、内口界面钉加栓桩、悬吊钛板等。
股骨侧最常见的固定方式是悬吊钛板； 胫骨侧最常见的固定方式则为外口界面螺钉固定。
