本文讨论了类风湿关节炎(RA)患者早期干预的重要性,并探讨了从临床前阶段开始采取预防措施的潜力。与1型糖尿病(T1D)相似,在临床发病前存在特异性自身抗体可以用于筛查两种疾病的高危人群。本文提出了有效预防RA的几个关键考虑因素:开发一种高通量多重电化学发光法(ECL)用于RA; 深入研究自身抗体的结合活性和动态随访;将遗传信息纳入自身抗体筛查方案; 研究具有独特剂量和持续时间的干预药物,类似于用于T1D预防的特普利珠单抗;使用利妥昔单抗和阿巴西普序贯靶向 B 细胞和 T 细胞的试验可能是探索 RA 高危个体的一种有价值的方法;考虑关节痛、自身抗体状态、遗传因素和环境风险等因素,开发准确的个体风险评估预测模型。人工智能的发展为构建这两种疾病的准确预测模型提供了新的途径。

Human gingival tissue-derived mesenchymal stem cells inhibit proliferation and invasion of rheumatoid fibroblast-like synoviocytes via the CD39/CD73 signaling pathway (Original Article)

研究发现,来自牙龈的间充质干细胞(GMSCs)在实验性关节炎模型中表现出抑制免疫和炎症反应的能力。本研究为了确定GMSCs是否能够抑制类风湿关节炎成纤维样滑膜细胞(RA FLSs)的增殖和侵袭。通过流式细胞术分析GMSCs表面标记,定量聚合酶链反应测量RA FLSs中基质金属蛋白酶和促炎细胞因子的mRNA表达。实验结果显示,GMSCs相对于人体皮肤成纤维细胞表达更高的CD39。在与GMSCs孵育后,RA FLSs中多种炎症相关基因的表达减少。体外实验证明GMSCs以剂量依赖性方式显著减少了RA FLS的增殖和侵袭能力,其中部分依赖于CD39/CD73信号传导。在II型胶原诱导性关节炎小鼠模型中,GMSCs输注延缓了关节炎的发病,并降低了关节炎评分。这些发现表明GMSCs对于抑制RA FLSs的侵略行为具有显著疗效,暗示了其在类风湿关节炎患者中可能具有治疗潜力。
