Bulletin of typical workplace accidents in Shanghai, September 2024























Overview

In September 2024, there were 27 fatal workplace accidents in Shanghai with 31 deaths.























Some Accidents in September


On September 2, an accident happened at the factory of Ounuoxin (Shanghai) Technology Development Co., Ltd. at No.1151, Guangtai Road. An operator was crushed by a machine and died from the injury.



On September 15, an accident happened at a construction site in Jiangwanzhen Neighborhood. A steel sheet on the ground was tangled in the treads of an excavator operated by a driver from Shanghai Kaiben Foundational Construction Co., Ltd. An operator standing nearby was hit by the sheet and died.



On September 23, an accident happened at No.30, Lane 38, Caoli Road, Fengjing Town. While a forklift was moving a machine on the site, the machine tipped over and struck a passing operator. The individual died from the injury.  



On September 29, an accident happened in the meeting room on the third floor of No.5 building, No.1896, Changtao Road. An operator from Hangzhou Tianli Construction Labor Service Co., Ltd. was testing the newly installed air-conditioner. The individual had an electric shock and died.



On September 30, an accident happened on the fifth floor of No.13 Building, Lane 981, Qinghewan Road. An operator from Zhenzhou Lemon Tree Decoration and Design Planning Co., Ltd. was adjusting the wires at the mezzanine floor. The individual had an electric shock and died. 























Workplace Accidents in Previous Septembers

“2022.9.5” Fall from Height Accident of a Nanjing Construction Material Company


Summary of the accident:

At about 2:05 pm, September 5, 2022, an accident occurred at a construction site in the Zhenru area. An operator of a Nanjing construction material company climbed over the guardrail to check the condition of the construction site. The individual accidentally fell to the ground and died. 


Direct cause:

The operator lacked safety awareness and broke the safety protocol to climb over the guardrail, which caused the accident. 


Indirect cause:

1. The Nanjing company did not take up its safety responsibilities and did not organize effective safety training among its employees. It did not request its workers to strictly follow the safety rules, did not inform the operators of the hazards, and did not teach them the protective measures. 

2. The construction company which was in charge of the site did not properly supervise third-party companies’ workers on the job site. It did not identify the hazards and did not stop unsafe behaviors of third-party workers.   


Punishment for the accident:

The Putuo District Emergency Management Bureau imposed an administrative penalty upon the Nanjing company. The Putuo District Construction and Administration Commission imposed penalties upon the construction company.


Recommendations:

1.Both companies should learn from this fatal accident, completely take up their work safety responsibilities, and implement an internal safety accountability system. 

2. The Nanjing company should conduct a comprehensive review of the safety lapses exposed by the accident. It should enhance workers’ safety awareness through rigorous training, implement strict internal safety rules, request all workers to follow the regulations and stop unsafe behavior. It should ensure all workers are fully acquainted with job site hazards, protective measures, and emergency procedures.

3. The construction company should improve its job site management and better supervise third-party workers. It should intensify its safety patrols and request all workers strictly follow safety regulations and standard operating procedures. It should identify hazards and eliminate risks quickly. 


“2023.9.3” Fall from Height Accident of a Shanghai Mechanical and Electrical Engineering Company


Summary of the accident:

At about 1:24 pm, September 3, 2023, an accident occurred at No.341, Tianshan Road. An air-conditioner repairer of a Shanghai mechanical and electrical engineering company fell from height while installing an outdoor unit. The individual died.


Direct cause:

The operator violated safety rules, did not wear any personal protective equipment and slipped from the sixth floor’s platform to the fifth floor. 


Indirect cause:

The Shanghai company did not take up its safety responsibilities, did not have a robust system of safety regulations, and failed to organize safety training. It did not identify and eliminate risks before the job began.


Punishment for the accident:

The Changning District Emergency Management Bureau imposed an administrative penalty upon the company.


Recommendations:

1. The company should learn from this fatal accident, implement an internal safety accountability system, and draft its internal safety regulations and standard operating procedures. It should rigorously carry out safety training and briefing, request all workers to correctly use personal protective equipment, and require everyone to strictly follow the safety rules. It should take up its safety responsibilities to effectively improve its safety management level.

2. The property management company of the building should also learn from this accident to improve its internal regulations. It should specify each department’s responsibilities and enhance its supervision over tenants’ safety works. Additionally, by requesting operators to use protective measures and fulfill their safety obligations, the company can mitigate potential risks and prevent future accidents.























Hazard warnings

Currently, many companies face pressure to meet production or sales targets. Some of these companies resort to risky measures, such as delaying essential machinery maintenance, rushing through inspection and repair processes, operating faulty equipment, and forcing workers to endure excessive workloads.

➤The increasing frequency of extreme weather events has led to heightened safety risks in transportation and outdoor operations. Fog and dense fog can easily cause vehicle rear-end collisions. Cold waves and strong winds can trigger wind disasters and capsize vessels. Freezing temperatures, rain, and snow can cause road accidents for food delivery workers and couriers. Low temperatures will require longer time for concrete to freeze. Inadequate support or premature removal of supportive structure can cause collapses at construction sites.

➤With falling temperatures and dry conditions, the demand for heating, electricity and gas will increase. Places like schools, temporary housing and nursing homes often use high-wattage electric appliances and gas appliances. These places are more likely to experience fires caused by overloading or forgetting to turn off equipment.
